Facial Plastic Surgery Clinic: Montreal, Quebec
From 217 to 1,651 First-Page Rankings
Montreal is one of the hardest markets in Canada for facial plastic surgery SEO. Patients search in both French and English, Google holds medical content to its highest YMYL standard, and established clinics have years of content head start. Breaking through requires more than standard SEO. It requires a methodology built for the complexity of bilingual healthcare search.
When we started working with this clinic in January 2023, the digital presence didn't match the clinical reputation. Three years later, it does.
Wins
Here is a snapshot of where this clinic ranks today for its most valuable keywords. These are real positions from SEMrush as of March 2026.
The Client
The clinic is Montreal's only cosmetic practice dedicated solely to the face. The surgeon introduced ultrasonic rhinoplasty to Quebec and pioneered the buccal fat removal and transfer procedure in the province. Clinically exceptional, but digitally invisible for non-branded searches.
The bilingual website needed to rank for procedure keywords in both English and French, across every service category. That meant building and optimizing dedicated pages for dozens of surgical and medispa treatments in two languages, each with its own competitive landscape.
The Baseline
When we started in January 2023, the site ranked for 1,152 total keywords, but only 217 sat on the first page. Daily organic clicks hovered around 50, mostly branded. The French side of the site was underperforming significantly. High-intent procedure searches in both languages, the kind that drive consultation bookings, were going to competitors.
The Strategy
Technical Foundation
Hreflang implementation across all pages, structured data (LocalBusiness, Service, MedicalOrganization schema), Core Web Vitals optimization, and clean bilingual URL architecture. Every technical decision was made to support scalable bilingual content.
Procedure Page Architecture with Contextual Density
70+ procedure pages built or rebuilt in both languages. Each one benchmarked against top competitors and engineered to exceed their contextual density score. Rhinoplasty alone required 8 dedicated pages (standard, revision, ultrasonic, ethnic, Middle Eastern, alar base, non-surgical, male). Multiply that across facelifts, eye surgery, chin procedures, injectables, laser treatments, and medispa services. In two languages. Every page was treated as its own competitive battlefield.
E-E-A-T Authority Signals
Board certification, Royal College fellowship, Stanford fellowship, published textbook, and peer-reviewed research woven into content and schema. Every procedure page attributed to a verified specialist. In YMYL healthcare, credentials are ranking signals, and this clinic had credentials most competitors couldn't match. We made sure Google knew it.
Local SEO
Google Business Profile optimization, NAP consistency across 70+ healthcare directories, bilingual review generation, and geo-targeted content for Montreal procedure searches. The goal was to own the Map Pack for high-value facial surgery queries in both languages.
Bilingual Compounding
French content treated as a standalone SEO asset, not a translation afterthought. Less competition in French meant faster rankings, which passed authority back to English pages through internal linking. Two languages compounding faster than one. This asymmetry gave the clinic an advantage that monolingual competitors simply do not have access to.
Blog-Driven Topical Authority
Comprehensive blog covering procedure science, recovery guides, cost breakdowns, technology explainers, and patient safety. All optimized with Contextual Density and internally linked to commercial pages. The blog captures patients still in the research phase and builds the topical authority that lifts every service page on the site.
The Results: 3 Years

Why This Matters
The growth wasn't linear. Year one built the foundation. Year two accelerated. Year three produced the steepest gains. That's the compounding effect of bilingual SEO done correctly, and it creates a competitive moat that monolingual competitors can't replicate without years of rebuilding.
Every month this clinic stays ahead, the gap widens. That's how compounding authority works.
Your Practice Has the Same Potential
This clinic started with 217 first-page keywords and 50 daily clicks. Today it holds 1,651 first-page positions, 455 top-3 rankings, and pulls in over 650 organic clicks per day. The starting position didn't determine the outcome. The strategy did.
The Contextual Density methodology, bilingual content architecture, and medical E-E-A-T framework behind these results are more refined today than when this campaign began in 2023. Every engagement since has sharpened the process.
If your practice has the clinical expertise but the search visibility doesn't reflect it, the gap is costing you patients every day. And every day you wait, competitors deepen their advantage.
